Looks like there’s a split between “native” designers and those of us who started as web tinkerers. It seems obvious to me that you need to start work within the parameters of the technologies you’re ultimately using (degrading gracefully for IE6), but the points in earlier comments about innovation are well made — sometimes you need to start with conceptualisation and visual design in order to push boundaries and come up with new solutions.
